Fragmented claim documents in. Trusted claims data out.

Claims teams spend too much time gathering and validating information across fragmented documents instead of making decisions. Intelligent Claims Operations helps insurers ingest, structure, validate and activate claims content in Box so teams can review faster, reduce risk and scale operations without adding headcount.

250,000+ Pages and checks a month for a major insurer, 87% arriving as physical mail
100% SLA adherence on fraud-related and claims integrity communications, scaled to 16 payer clients

The short answer

How can insurers reduce claims review time and risk from fragmented documents?

Insurers reduce claims review time by capturing claim documents from every channel into one process, classifying and normalizing them before an adjuster opens the file, extracting and validating the data inside, and delivering the result into Box and core claims systems automatically. Claims operations handled this way means adjusters spend their time deciding, not gathering and validating information by hand.

Intelligent Claims Operations is the ingestion, classification, validation and delivery of claim documents, forms, records, photos and estimates an insurer collects across the life of a claim, structured into a governed, searchable record in Box and fed into core claims systems and everything downstream of them.

Who it’s for

Claims operations, compliance/SIU and adjuster teams at insurance carriers.

What changes

Adjusters review a validated, structured claim file instead of gathering and keying it by hand.

A claim file, still being assembled

Where does claims documentation break down?

It breaks down in the gap between a document arriving and an adjuster being able to trust what it says.

From a claims operations desk

A claim packet arrives bundled together: a form, a repair estimate, two photos and a fax cover sheet, all scanned as one file. An adjuster has to split it apart before they can work any of it.

Meanwhile, a different claim’s supporting documents came in through the portal, the eForm and a follow-up email, each with its own metadata, none of it matching. The adjuster keys the same policyholder information into the system three times, once per source.

Then a compliance request comes in on a claim from eight months ago. Finding every document that supported that determination means checking four systems and a shared drive, hoping nothing from that file ever got misplaced.

The document pattern

Why do claims operations create rework?

Every claim moves through three stages: fragmented intake, review bottlenecks, and compliance and audit risk. Each stage has a friction point, and each friction point has a downstream cost.

Stage 01

Fragmented intake

Friction

Claims arrive via email, portals, eForms, fax, scans and paper, often with forms, records, photos and estimates bundled together.

Cost: Mixed claim packets

A single claim file has to be pulled apart before anyone can work it.

Stage 02

Review bottlenecks

Friction

Adjusters spend time finding, keying and validating claim details by hand, with metadata varying across documents, systems and lines of business.

Cost: Manual data entry

Claims admin rework eats into the time adjusters should spend deciding, not sorting.

Stage 03

Compliance and audit risk

Friction

Missing or conflicting information slows review and decisioning, and unstructured records are hard to produce quickly.

Cost: Audit exposure

A fraud review or compliance request needs a record fast, and the team can’t say how confident they are in what they’ll find.

At every stage above, a person is still reconciling the claim by hand.

Why now

Why is claims documentation harder to manage right now?

Regulators expect a defensible record, and claim volume isn’t slowing down to make that easier.

Claim file documentation is an exam standard, not a courtesy

The NAIC Market Regulation Handbook’s claims standard checks that claim file documentation is sufficient to reconstruct pertinent events and support the final determination.

Retention requirements vary by state

NAIC’s model sets a baseline of the current year plus three years for closed claim files, while New York requires six years and other states extend further, which makes a governed, searchable record more valuable the longer a file has to be kept.

Claim volume keeps outpacing headcount

Carriers report claim volume growing faster than their teams’ ability to keep up without adding headcount, the exact pressure that makes fragmented, manual intake unsustainable.

By role

Answers for everyone at the table

A claims operations decision crosses claims operations, finance, compliance and the adjusters doing the work. Each role starts with a different question.

Outcomes

How you’ll measure success

Adjusters start from a validated claim file, so decisions move faster and hold up under review.

Faster claim decisions

Teams review from trusted, structured claim data instead of fragmented documents.

Lower exception rate and smaller review queues

Confidence scoring and rule checks route only true exceptions to a human reviewer.

Reduced manual data entry and rework

Less sorting, splitting, indexing and correcting files by hand.

Improved compliance and audit readiness

Every claim record carries its source and handling history, ready for a fraud review or compliance request.

Real-world results

How has this worked for an insurer?

Two real facts from the same DataBank engagement, run for a major insurance company.

Insurer experience

Outsourced mailroom operations

Challenge

Claims correspondence arrived through dozens of channels, with the majority still physical mail.

Result

DataBank runs the fully outsourced mailroom: 250,000+ pages and checks a month, 87% arriving as physical mail, scanned, extracted and delivered directly into core claims and CRM systems daily.

Insurer experience

Scaled across 16 payer clients

100% SLA adherence maintained on fraud-related and claims integrity communications, while scaling to 16 payer clients on one standardized framework.

Security and compliance

How is claims data kept secure and auditable?

Claim records stay under documented custody from receipt to the governed record, with access controls, audit trails and retention enforced on the platform.

Chain of custody

Every handoff logged, paper and digital, from receipt to the governed claim record.

Access controls

Role-based permissions, so staff see only the claims and lines of business they work.

Audit trails

Every view, edit and routing step recorded, for both routine review and SIU investigations.

Retention

Retention schedules applied on the platform and enforced by rule, matching the states and lines of business you operate in.
